Exploration Program Dates and Times:	Session 1: July 5 – July 29, 2011 8:45 a.m. – 10:15 a.m.

	Session 2: July 5 – July 29, 2011 10:30 a.m. – 12:00 noon

Program Location:	Sayville Middle School

When registering please remember to fill out all information requested on the registration information page.

A SEPARATE REGISTRATION FORM SHOULD BE COMPLETED FOR EACH CHILD ATTENDING

PLEASE INCLUDE A SECOND CHOICE WHERE INDICATED
