Primary Registration

PRIMARY MAIL IN REGISTRATION INFORMATION

Complete Registration Form and Medical Form (Download below).
Make check payable to SCOPE. Write child’s name and phone number on check.
Credit Card payments are also accepted. (Download below).
Mail in completed Registration Form, Medical Form, Immunization Record, Birth Certificate (if required) and check or credit card form to: SCOPE/Manhasset Summer Center, 100 Lawrence Avenue, Smithtown, NY 11787
All Pre-K, Pre-1 and Pre-2 registrations must include a copy of immunizations. This copy is in addition to that required in September.
All Pre-K students and students not enrolled in Manhasset Public Schools or St. Mary’s must include copy of birth certificate.

A SEPARATE REGISTRATION SHOULD BE COMPLETED FOR EACH CHILD ATTENDING

In order for your child’s registration to be complete, the Medical Form (download above) must be completed and returned to SCOPE @ SCOPE/Manhasset Summer Center, 100 Lawrence Avenue, Smithtown, NY 11787.
