INTERMEDIATE MAIL IN REGISTRATION INFORMATION

Registration for courses is on a first come, first served basis except for Pre-6 students who will be given priority.

Consult the schedule of courses and prepare a schedule for your child.

Complete Registration Form, Program Card, and Medical Form (Download below).
Make check payable to SCOPE. Write child’s name and phone number on check.
Credit Card payments are also accepted. (Download below).
Mail in completed Registration Form, Medical Form, Immunization Record, Birth Certificate (if required) and check or credit card form to: SCOPE/Manhasset Summer Center, 100 Lawrence Avenue, Smithtown, NY 11787
All Pre-3, Pre-4, Pre-5 and Pre-6 registrations must include a copy of immunizations.

A SEPARATE REGISTRATION SHOULD BE COMPLETED FOR EACH CHILD ATTENDING
