- A SEPARATE REGISTRATION FORM SHOULD BE COMPLETED FOR EACH CHILD ATTENDING
- YOU WILL BE PROMPTED TO ENTER SECOND CHOICES FIRST - PLEASE INCLUDE A SECOND CHOICE WHERE INDICATED
